OBJECTIVE: To describe the nutritional and clinical status of Guatemalan pediatric patients attending the hemodialysis (HD) unit. METHOD: This was a cross-sectional study that evaluated Anthropometry "A", Biochemistry "B", Clinical "C" and Dietary Intake "D" in patients from 8 to 17 years of age with chronic kidney disease CKD who attend HD Hospital Roosevelt/FUNDANIER of Guatemala, including 30 cases. Albumin and cholesterol are used to identify one of four criteria based on Protein Energy Loss (PEW). The nutritional alterations in micronutrients were determined by the clinical signs referenced by physical examination. Daily caloric intake was restricted with the 24-h reminder on HD and non-HD days. RESULTS: 60% have a moderately low height-for-age ratio, followed by 17% with a moderately low height-for-age ratio. 70% had a normal BMI. Albumin and cholesterol were normal, 26% had micronutrient deficiencies. Most of the patients had an energy intake lower than recommended, 273 Kcal deficient on the days they attended HD. CONCLUSION: BMI measurement alone can mask malnutrition in pediatric CKD patients living in low-to-middle-income countries (LMIC). We propose that the nutritional evaluation be comprehensive and designed to include possible parameters.
